Our goal is to provide care for the entire community of Clarence-Rockland within a few years thanks to the arrival of new healthcare professionals as well as a few family doctors.

To join the CRFHT you will be required to complete a registration form.

Our responsibilities:

• Longer hours of operation at night and on weekends in case of emergency (by appointment).
  
  
• Telephone Health Advisory Service (THAS ) for help after hours - 1-866-553-7205
  
  
• Access to other healthcare professionals.
  
  
• A more flexible system of operation.
  
  
• Thanks to our computerized system, automatic reminder calls for all patients regarding certain preventive exams (Pap test, cholesterol screening.)
  

• Consult the CRFHT for ALL health issues that are not extremely urgent or do not involve hospital treatment by making an appointment at the clinic or contacting the THAS when the clinics are closed.
  
  
• Do not visit walk-in clinics such as the emergency clinic in Orleans or the 'Appletree' clinics without consulting us first. Please note that this does not apply to services rendered by specialists, care received in the emergency room of a hospital or a hospital stay. By respecting this request, you can rest assured that your doctor is constantly kept abreast of your state of health allowing her/him to treat you better.
  
  
• Book follow up appointments as suggested by your doctor to ensure good continuity of care.
  
  
• Bring all your medications to your appointment.
  
  
• Provide at least 24 hours notice if you can't come to an appointment so we can give it to another patient.
