New Patient Registration

We are pleased to welcome two new family physicians, Dr. Sophie Turcotte and Dr. Zaida Altamirano, who are joining our team.

In this context, the Clarence-Rockland Family Health Team is accepting a total of 2,050 new patients residing ONLY in the City of Clarence-Rockland (Rockland, Bourget, Cheney, Clarence, Clarence-Creek, Hammond, St-Pascal Baylon) as well as Wendover. No exceptions apply.

Please note:

  • Registration is completed online only through the Ocean appointment booking platform.
  • The registration appointment is an administrative visit with our reception team. It is used to open your file and schedule your first appointment with your doctor. Unfortunately, no medical questions can be discussed or addressed during this visit.
  • Each family member aged 18 or older must book a registration appointment.
  • Each patient must have a valid OHIP health card as well as proof of residency.
  • If you do not provide valid proof of residency dated within the last 3 months at your appointment, your registration cannot be completed and you will need to book a new appointment (if you meet the eligibility requirements).
  • If your registration is accepted, new patient appointments will be scheduled for all family members, including children.

Important:

A registration appointment does not guarantee acceptance as a patient. After your appointment, additional review and consultation with management may be required before a final decision is made. If further review is needed, your file may be placed on a callback list while your application is being processed.

We have a zero-tolerance policy for any aggressive, abusive, threatening, or violent behaviour toward staff, patients, or visitors. We reserve the right to ask anyone to leave the premises if their behaviour is considered inappropriate, disruptive, or unsafe.

The number of weekly appointments is limited. Once capacity has been reached, no available dates will appear in the calendar. A new batch of appointments is released every Monday around 9:00 a.m.

If no dates are available, this does not mean that we are no longer accepting new patients, but simply that the weekly capacity has been reached. We kindly ask you to try again at the next opening, or to check periodically for cancellations, as appointments may become available at any time if other patients cancel.

We kindly ask that you do not call the clinic about this, as our administrative team is unable to complete registrations by phone.
