New Patient Registration

We are pleased to welcome two new family physicians, Dr. Sophie Turcotte and Dr. Zaida Altamirano, who are joining our team.

In this context, the Clarence-Rockland Family Health Team is accepting a total of 2,050 new patients residing ONLY in the City of Clarence-Rockland (Rockland, Bourget, Cheney, Clarence, Clarence-Creek, Hammond, St-Pascal Baylon) as well as Wendover. No exceptions apply.

Please note:

  • Registration is done exclusively through our online form (see below)
  • An application must be submitted for each family member, including all children regardless of age
  • Patients must have a valid OHIP health card
  • The form will remain open for 30 mins once you start your application
  • The form is available up to a maximum capacity of 100 patients per week
  • Once this capacity is reached, the form is temporarily disabled
  • It is reactivated every Monday at 9:00 a.m

If the form is no longer accessible, this does not mean that we are no longer accepting new patients, but simply that the weekly capacity has been reached.
We invite you to try again when the form reopens.

Need help accessing the form? A family member or friend can assist you in completing the online registration.

We kindly ask that you do not call the clinic regarding this process, as our administrative team is unable to complete registrations by phone.

Important: Proof of Address Required

⚠️  Before proceeding with the application, please ensure you have a saved copy of valid proof of address dated within 3 months of the date you submit this application.

This means:

  • The document must be no older than 3 months on the day you apply 
  • Future-dated move-in documents are not accepted ( ex: Agreement of Purchase and Sale (APS) or Residential Tenancy Agreements (Ontario Standard Lease Forms )
  • Documents older than 3 months will not be accepted 

Each document must clearly show:

  • Full legal name as stated on your health card (for each person aged 18 and over) 
  • Current address in the eligible area 
  • A visible issue or statement date within the 3-month window 

✅ Accepted documents may include:

  • Recent utility bill (electricity, gas, water)
  • Bank or credit card statement 
  • Government-issued correspondence dated within 3 months 

⛔ Not accepted:

  • Health card, permanent residency cards or passports (no address) 
  • Driver’s licence (does not confirm issue date) 
  • Tax slips (e.g., T4s)
  • Any document missing name, address, or a valid date 

⚠️ If your proof of address does not meet these requirements, your application will not be processed and you will not be contacted by the clinic

⚠️ If you do not reside within the eligible service area, your application will not be processed and you will not be contacted by the clinic.

*** If you have not received a call within a three-week period, you can assume your application did not meet the above criteria and resubmit the proper documents, if applicable. ***

Medical costs not covered by the Ontario Health Insurance Plan

Some services offered by the CRFHT are not covered by the Ministry of Health (OHIP). Please note that fees may be subject to change without notice.

We accept cash, Interac, VISA or MasterCard.

If you provided us with an email address during your last clinic visit or if we already have your email on file, we can also send you an electronic invoice.

Once your fees are paid, we can send you the copies your documents (medical certificate, forms, chart copies) by email. Exceptions apply for certain forms.
